Monash University supplementary information form for VTAC applicants 2009-2010

Welcome to the 2009-2010 online Monash University Supplementary Information Form for some Monash courses* participating in the VTAC application process.

Only proceed with this form if you have a course or courses on your VTAC preference list that state in their Extra Requirements (in the course entry of the VTAC Guide or on VTAC course search) that you must complete the online Monash University Supplementary Information Form.

*Note*: Applicants to Bachelor of Nursing at the Peninsula Campus (VTAC Course codes 29371 and 29373) are not required to submit a Monash University Supplementary Information Form or attend an interview as suggested in the Selection mode of the Nursing (Peninsula) course entry in VTAC Guide 2010.

Applicants must have submitted a VTAC application and have a current VTAC ID to complete the form.

Applicants must read the instructions on the first page of the form carefully.

Applications are to be completed in one sitting. Where courses require long or considered responses to their questions, applicants are advised to draft these responses in a word processing program and cut and paste the response into the online form. The long response questions can be found on the Personal statements link.

Applicants with queries regarding the Monash University Supplementary Information Form should submit a question to the Admission Unit using an online Contact Form. All queries will be responded to within two working days.

* Monash University courses requiring the completion of this form include most Business and Economics courses, Sport and Outdoor Recreation, and some Graduate Diploma of Education courses, Emergency Health (Paramedic), Health Sciences/Social Work, Nursing (Gippsland), Nursing and Rural Health Practice, Occupational Therapy; Nutrition and Dietetics; Radiography and Medical Imaging; and Social and Community Welfare.  Double degrees including these courses may also require completion of this form (check the extra requirements section of the course entry in the VTAC Guide or via the online VTAC course search).
