ICT Shared Services - IT Assessment Documentation Request List

The following is a data request list for information for the ICT Shared Service project to document and validate all ICT services in the University.

INFORMATION TECHNOLOGY - Infrastructure
1.

Asset registers available:

  1. List of assets that are tagged with barcodes. (Equipment lists)
  2. List of items that are under $5000.
  3. List of all application software including developer and vendor,
    (please exclude University site licences).

 

2.

Does your Faculty/Division manage any servers?

  1. If you operate a data centre, please provide documentation
  2. If no data centre, where are the servers located?

 

3.

Are there one or several standard desktop and server images?

  1. Please provide documentation on standard desk and server images.

 

4.

Information regarding your refresh program for desktops and servers

 

5.

Server, application and network availability reports (12 months).

 

6.

Network diagrams - WAN/LAN including permanent/temporary external connections.

(For Networks additional to or separate from the University Infrastructure)

 

7.

Current infrastructure capacity assessment

  1. capacity planning (12 month forecast).

 

8.

Infrastructure for development, test and production environments.

 

9.

Infrastructure Projects (Any Signed or pending contracts with the following):

  1. Infrastructure vendors;
  2. Service providers;
  3. Consulting firms; and
  4. Sub-contractors.

 

INFORMATION TECHNOLOGY - Planning
10. Faculty/Division Technology Strategic plans.  
11. Faculty / Division IT Operating plans,  
12. Faculty/Division IT Budgets  
INFORMATION TECHNOLOGY - Services, Process and Procedures
13. Help desk or desktop assessment (e.g., client satisfaction surveys, service level reports etc).  
14.

Guidelines, policies, working instructions, checklists and Standard Operating Procedures on:

  1. Data protection;
  2. IT security;
  3. Privacy;
  4. Services;
  5. Asset/information classification;
  6. Change management;
  7. Project Management;
  8. Procurement and setup of workstation and application access;
  9. Asset management;
  10. Technical standards (definition and status of implementation); and
  11. Service Level Agreements.
 
15. List of any Customised IT services or tools  
16. Reviews performed - primary focus IT systems and Data Management (e.g., penetrations test or number of problems logged in the help desk system)  
17.

Disaster recovery and business continuity plans including

  1. identification of critical systems and
  2. evidence of testing.
 
18. List of active, upcoming and backlogged IT projects (12 month period)  
19. Reporting and performance measurement reports that you provide to the Faculty /Division management or clients (for 12 months)  
INFORMATION TECHNOLOGY - Human Resources
20. IT organisational chart  
21. Number of FTEs per service (include HEW level and location)

22. Position descriptions for each position type (group like positions together).  Or a paragraph describing the role and how many staff perform that role.  
23. Specialist skills available (regards customised IT services or tools in 14)
