Reporting and recording sick leave

It is the responsibility of the staff member to submit to the Head of Administrative Unit an "Application for Leave" form to cover each period of absence due to illness; and to provide a medical certificate if appropriate.

Sick leave will be recorded in full days or fraction thereof.

The period of leave is to be recorded as commencing on the next University working day after duties cease and as ending on the University working day prior to the staff member's return to duty, except where leave commences or ends during a day, in which case that day is to be included.

The amount of leave taken is to be recorded as the sum total of the University working days/fraction of days ordinarily worked by the staff member, excluding any University holidays, during the leave period.

It is the responsibility of the Head of Administrative Unit or of an authorised delegate to ensure that formal sick leave applications are submitted and that forms, authorised or otherwise endorsed as appropriate, are sent to the Human Resources Division together with medical certificates if supplied. If a medical certificate has been requested and has not been provided by the staff member, the period of absence will be as leave without pay or, at the request of the staff member, shall be deducted from the annual leave credits of the staff member.

Please refer to Clause 37 - Application of Sick Leave and Schedule 5(b) of the Monash University Enterprise Agreement (Academic and General Staff) 2005 or, where applicable, the provisions of the relevant AWA Terms and Benefits Policy.
